Question: What is the difference between vital capacity and peak flow? We report CPT 94150 for vital capacity. Should we also use the code for peak flow?

Peak flow and vital capacity are different procedures that use unique equipment. Vital capacity is part of a spirometry measurement that assesses flow over time. This expensive piece of equipment gives you more complex and valuable results than a peak-flow measurement.
No CPT code exists for a peak expiratory flow rate measurement from a handheld peak-flow meter. You should include the test in your diagnostic workup and medical decision-making part of the E/M service. If you properly document the procedure, the work could lead you to report a higher-level E/M service (such as 99212-99215,
Office or other outpatient visit for the evaluation and management of an established patient ...).
You should report vital capacity with the appropriate spirometry code. For example, spirometry including graphic record and measurement of vital capacity and expiratory flow rate would warrant 94010 (
Spirometry, including graphic record, total and timed vital capacity, expiratory flow rate measurement[s], with or without maximal voluntary ventilation). If you perform a vital capacity screening test, you should use 94150 (Vital capacity, total [separate procedure]).
