Break Down Globulin Injection for Some Payers

CMS replaces J1567 with 4 product-specific Q codes If your payer decides to follow CMS' HCPCS level-II quarterly update, you'll be able to specify what immune globulin you give. As of July 1, Medicare Part B will not pay for J1567 (Immune globulin, liquid Injection, immune globulin, intravenous, non-lyophilized [e.g. liquid], 500 mg). If a private payer no longer accepts the J code, use the new product-specific Q code as follows: • Q4087 -- Injection, immune globulin (Octagam), intravenous, non-lyophilized (e.g. liquid), 500 mg • Q4088 -- Injection, immune globulin (Gammagard Liquid),intravenous, non-lyophilized (e.g. liquid), 500 mg • Q4091 -- Injection, immune globulin (Flebogamma), intravenous, non-lyophilized (e.g., liquid), 500 mg            • Q4092 -- Injection, immune globulin (Gamunex), intravenous, non-             lyophilized (e.g., liquid), 500 mg.
