Multiple Slides for Bronchial Brushing

Question: Which code(s) should we use for a bronchial brushing specimen that involves seven slides received from the bronchoscopy unit, already stained and read? Should we use 88162 to reflect that the specimen involves more than five slides?

Answer: Code 88162 (Cytopathology, smears, any other source; extended study involving over 5 slides and/or multiple stains) should not be used in the scenario you describe. This code is part of the 88160 family of codes that describe cytopathology smears from "any other source," meaning any source other than those listed in the other cytopathology codes. The other cytopathology codes describe the source as "cervical or vaginal" (88141-88155), and "fluids, washings or brushings, except cervical or vaginal" (88104-88107). The 88160 family is used only for specimens such as sputum, nipple discharge, or touch preps.

When the specimen involves direct smears from a bronchial brushing, the appropriate code is 88104 (Cytopathology, fluids, washings or brushings, except cervical or vaginal; smears with interpretation). Notice that the definition includes "smears" (plural) but does not designate a number.

Your question implies that the slides were prepared at your facility, but it is important to note that the coding would be different if the prepared slides were referred for a consultation from another, outside source, such as another independent pathologist or facility.
