Match Test Methods, Specimen Sources to Codes Using This Guide

Don't search in vain through CPT to find the proper code for a Helicobacter pylori test. Instead, use the following chart to easily select the code if you know the specimen source and lab method. * (QW) indicates that if you operate a waived-status lab under the clinical laboratory improvement amendments (CLIA) and perform a CLIA-waived test approved for this code, you must report the code with modifier QW (CLIA-waived test).
