Question: When we bill 92537, does that code represent the four irrigations as one unit or do we bill four units? Delaware Subscriber Answer: You should bill procedure code 92537 (Caloric vestibular test with recording, bilateral; bithermal (ie, one warm and one cool irrigation in each ear for a total of four irrigations)) just once to represent all four irrigations (one warm and one cool in the left ear, and one warm and one cool in the right ear). In cases where the physician performs just one irrigation per ear (so, two irrigations total), you will instead submit the claim with 92538 (Caloric vestibular test with recording, bilateral; monothermal (ie, one irrigation in each ear for a total of two irrigations)).
