The reference time for 99353 (Inpatient consultation for a new or established patient, which requires these 3 key components: a detailed history; a detailed examination; and medical decision making of low complexity&) is 55 minutes, so the documentation of at least 85 minutes will qualify. If he documented 90 minutes -- and what he was doing with the patient -- you can bill 99253 and +99356.
