Question:  What are the correct procedure and diagnosis codes for hemangioma excision by laser under microscope?

Answer: For the procedure code, choose either 17110 (Destruction [e.g., laser surgery, electrosurgery, cryosurgery, chemosurgery, surgical curettement], of benign lesions other than skin tags or cutaneous vascular proliferative lesions; up to 14 lesions) or 17111 (… 15 or more lesions), based on the number of lesions removed. 

Select the diagnosis from the ICD-9 code family 228.0x (Hemangioma …), according to the anatomic site. When you begin using ICD-10 in October, the diagnosis will change to D18.0x and will again have several options based on site.
