Put NPI in 24J on CMS-1500

You should enter the National Provider Identifier (NPI) in box 24J, not 26J. 2008 Otolaryngology Coding Alert Vol. 10, No. 5 incorrectly listed box 26J, which does not exist on the CMS-1500 (08-05 form).
