Using Modifier -57

Question: Is a modifier used when undertaking non-surgical as well as surgical treatment in the global period?

Answer: The -57 modifier (decision for surgery) would be appropriate for any E/M service performed the day before or the day of a major procedure (any procedure with a 90-day follow-up), whether the treatment is an open or closed procedure. Fracture care, whether open or closed, is considered a surgical procedure for this purpose. For commercial carriers that have global periods less than 90 days, but longer than the 0- or 10-day global period, verify with them whether this modifier is recognized.
