No Disruption = No 99058

Question: If a patient calls in with an emergent issue, but the surgeon just uses an available slot for the visit, can I bill 99058 in addition to his E/M service? Virginia Subscriber Answer: No, you should report 99058 (Service[s] provided on an emergency basis in-the office, which disrupts other scheduled office services,-in addition to basic service) only when the orthopedic surgeon must interrupt his normal course of scheduling to treat the patient in the office. CPT revised 99058's descriptor in 2006 to emphasize that the service must disrupt other scheduled services for you to report the code.
