Get Specific Details on Prosthetic Shoulder Removal

Question: If we remove a prosthetic shoulder implant, how can we code for it? Also, are there separate coding opportunities if we perform debridement and/or synovectomy?

Answer: You can report the removal, but you cannot code separately for debridement or synovectomy, as the following code descriptors will make clear.

Do this: Go back and check the notes on the removal and get answers to these questions:

  • Did the surgeon remove the humeral component; the glenoid component; or both components?
  • Did the surgeon remove the prosthesis from the right or left side?

Then you can make your CPT® code and modifier choices. You should choose either:

  • 23334 (Removal of prosthesis, includes debridement and synovectomy when performed; humeral or glenoid component) for removal of the humeral or glenoid component; or
  • 23335 (… humeral and glenoid components (eg, total shoulder)) for removal of both the humeral and glenoid component.

Also, append modifier RT (Right side) appended to 23334/23335 for removal of a right shoulder prosthesis; or modifier LT (Left side) appended to 23334/23335 for removal of a left shoulder prosthesis.
