Report Unlisted-Procedure Code for Topaz Procedure

Question: Our surgeons have started documenting "Topaz procedures" for elbow and knee surgeries. Is there a code for this surgery?

Answer: The Topaz procedure involves a process called "Coblation," which allows the surgeon to use radiofrequency energy to remove or shrink the patient's soft tissue volume.

CPT does not include a code for the Topaz procedure, so you should report this service as an unlisted procedure 24999 (Unlisted procedure, humerus or elbow) for elbow procedures or 27599 (Unlisted procedure, femur or knee) for knee procedures. Include a copy of your operative report with the claim, along with a letter explaining that you performed the Topaz procedure.

Because Coblation is relatively new, some payers still consider it investigational and may not reimburse for it, so always get an insurance prequalification letter before you perform the Topaz procedure.
