Reader Question:

Know the Bundling of Adductor Tenotomy and Total Hip

Question: Our surgeon performed a conversion of hemiarthroplasty to a total hip but only needed to insert the acetabular component as he was able to utilize the femoral stem. He also performed an adductor tenotomy -- 27000 (Tenotomy, adductor of hip, percutaneous [separate procedure]). Would this be bundled?Florida SubscriberAnswer: You report 27132 (Conversion of previous hip surgery to total hip arthroplasty, with or without autograft or allograft) for the conversion to total hip from previous hip surgery. You append modifier 52 (Reduced Services...) because your surgeon only did an acetabular component. According to CCI edits, code 27132 is not bundled with 27000. It is however bundled with 27001 (Tenotomy, adductor of hip, open). Check the operative note to confirm the open tenotomy.
