Question: How should I code for an arthroscopic rotator interval closure?

Answer: As with many arthroscopic procedures, the only code choice is 29909 (unlisted procedure, arthroscopy). But to make a case for reimbursement, submit the claim with a KISS letter identifying an analogous code -- one that describes a similar procedure and about the same amount of work as the surgery performed. In this case, it appears that an open rotator cuff repair code 23410 (repair of ruptured musculontendinous cuff [e.g., rotator cuff]; acute); or 23412 (... chronic) is the most appropriate analogous code because the lower portion of the subscapularis tendon was repaired.
