CCI or AAOS?

Question: The surgeon performed a Bostrom procedure (27698, repair, secondary, disrupted ligament, ankle, collateral [e.g., Watson-Jones procedure]) and also tenolysis, multiple flexors (27681-51, tenolysis, flexor or extensor tendon, leg and/or ankle; multiple tendons [through separate incisions]). The AAOS Global Data book indicates that 27681 is included with code 27698. However, per CCI, the codes are not bundled. Should I follow CCI edits or AAOS edits?

 Answer: The AAOS Global Service Data guide seems hard to argue with because a committee of orthopedic surgeons developed the guidelines. In this era of intense focus on compliance, it would be advisable for your physicians to designate one set of edits as the official guidelines for your practice. Bouncing back and forth between the CCI and AAOS edits may not be viewed kindly in the event of an audit. If an orthopedist feels that a particular edit in the AAOS book is wrong, he should take it up with the AAOS committee on CPT coding.
