Assistant Surgeon

Question: We often use orthopedic residents from an area program, but when they arent available our physicians assist each other. Which modifier should I use when this happens?

Answer: Use modifier -82 (assistant surgeon [when qualified resident surgeon not available]) when one physician has to assist another because the resident either is not qualified or is not available. Your physicians may operate at several different hospitals and/or ambulatory service centers, and not all have residency programs. When the doctor is operating at a hospital that is part of the residency program and no resident is available, append modifier -82 on the claim for the surgeon who is assisting. The carrier may require supporting documentation, so be sure the operative report lists this physicians name as the assistant surgeon. Reimbursement for the assistant surgeon will vary depending on the carrier.
