Question: Should I append modifier 50 to 67820 for a bilateral procedure?

Answer: Modifier 50 (Bilateral procedure) is generally used on procedures on paired organs like the eyes. However, CPT® code 67820 (Correction of trichiasis; epilation, by forceps only) describes a procedure done on the eyelids, which are not paired organs — there are four eyelids, not two. Medicare prefers that you use modifiers E1-E4 to designate eyelids:
