Massachusetts Subscriber Answer: Report code 65426 (Excision or transposition of pterygium; with graft). You cannot code separately for the amniotic membrane transplantation (AMT). Since October 2004, the AMT, represented by CPT code 65780 (Ocular surface reconstruction; amniotic membrane transplantation), has been bundled as a component of 65426.
