Answer: CMS offers a jurisdiction list that specifies this information.

The list is at www.cms.hhs.gov/transmittals/downloads/R893CP.pdf. More information is at www.cms.hhs.gov/MLNMattersArticles/downloads/MM4363.pdf. CMS plans to post an Excel file with the jurisdiction data at www.cms.hhs.gov/center/dme.asp.

What you'll find: The transmittal offers a chart that includes the HCPCS code, description, and jurisdiction. For example:
