You should attach modifier 25 (Significant, separately identifiable evaluation and management service by the same physician or other qualified health care professional on the same day of the procedure or other service) to the E/M code to indicate that the E/M service was separately identifiable from the injection if the documentation supports the definition of the modifier and work beyond the typical pre- and post- procedural service was provided. When your physician evaluated and managed the patient for a problem that was not in any way related to the allergy immunotherapy, you are justified in using a modifier to separately report the codes with compensation for both the codes. When the physician provides one injection for immunotherapy, you should assign code 95115.
