Select This Code for Sex-Cord Stromal Tumors

Question: We received a report from the path lab that said “right ovarian cyst resection. Sex-cord stromal tumor, consistent with Sertoli- Leydig tumor.” What code should I assign for this?

Answer: Even though ICD-10-CM guideline IV.H tells you that you should “not code diagnoses documented as ‘probable,’ ‘suspected,’ ‘questionable,’ ‘rule out,’ or ‘working diagnosis’ or other similar terms indicating uncertainty,” the Alphabetic Index points you to the same place for both the sex-cord stromal tumor diagnosis and the “consistent with” Sertoli- Leydig tumor diagnosis.

To find the correct code, you should first go to Tumor in the Alphabetic Index. There, you’ll find the entry for sex cord (stromal), which directs you to “see Neoplasm, uncertain behavior, by site.” This will lead you to D39.11 (Neoplasm of uncertain behavior of right ovary).

Remember: The D39.1- codes contain an instruction for you to use an additional code to identify any functional activity. For ovarian tumors, that means adding codes that document the influence their hormonal activity has on other body functions such as menstrual cycles, ovulation, and reproduction.
