Question: Can we take stage of the malignancy as an indicator of the severity or progression of disease? Can we consider this as severity in the History of Present Illness component of an E&M service when the patient presents with no symptoms, i.e. when the patient reports for a follow-up visit?

Answer: In a patient with cancer, you will consider ‘stage’ as indicator of extent of underlying malignancy. This implies the ‘severity’ component in history of present illness. You can use the stage of the malignancy as an HPI element.
