Ohio Subscriber Answer: When your physician administers vitamin K injection, you look to HCPCS code J3430 (Injection, phytonadione [vitamin K], per 1 mg). One unit for every mg: You submit one (1) unit of J3430 for every 1mg of vitamin K ordered and administered to the patient. Administration code: Also report 96372 (Therapeutic, prophylactic, or diagnostic injection [specify substance or drug]; subcutaneous or intramuscular) for administration of the injection.
