Answer: When your physician administers vitamin K injection, you look to HCPCS code J3430 (Injection, phytonadione [vitamin K], per 1 mg).

One unit for every mg: You submit one unit of J3430 for every mg of vitamin K ordered and the patient received.

Administration code: Also report 96372 (Therapeutic, prophylactic, or diagnostic injection [specify substance or drug]; subcutaneous or intramuscular) for administration of the injection.
