If No Intrauterine Contents, Use This D&C Code

Question:  I am having an issue with this procedure. The name of the procedure is “Left salpingectomy and removal of ectopic pregnancy as well as D&C.” I am using CPT® 59151. The American Congress of Obstetricians and Gynecologists (ACOG) states that the D&C can be reported separately with either CPT® 58120 or 59812. Would you advise which would be most appropriate for a patient who had irregular bleeding and no intrauterine pregnancy? I’m confused by ACOG stating CPT® code 58120 may be appropriate when it should be used for nonobstetrical.

Answer:  The ACOG coding committee has always thought that the 58120 (Dilation and curettage, diagnostic and/or therapeutic [nonobstetrical]) code is correct when there are no intrauterine contents at the time of the D&C. If there were none, that code would be correct. If there was, then you should use 59812 (Treatment of incomplete abortion, any trimester, completed surgically).
