Clobber Co-Surgeon for TAH/MMK

Question: My ob-gyn performed a total abdominal hysterectomy (TAH) (58150) with abdominal enterocele repair (57270-51). A urologist came in and did a Marshall-Marchetti-Krantz (MMK) procedure, then closed the abdomen. The ob-gyn went back and did an A&P repair vaginally (57260-59). Are these the correct codes for the procedures performed?

Answer: Because the ob-gyn and urologist are co-surgeons for the TAH/MMK, you should report 58152-62 (Total abdominal hysterectomy [corpus and cervix], with or without removal of tube[s], with or without removal of ovary[s]; with colpo-urethrocystopexy [e.g., Marshall-Marchetti-Krantz, Burch]; two surgeons).

Then the ob-gyn will also report 57265-51 (Combined anteroposterior colporrhaphy; with enterocele repair; multiple procedures), which includes all the other procedures he did. This code does not specify the route, but that does not matter. Code 57265 covers all three procedures. If you look at the National Correct Coding Initiative edits, you'll see that NCCI permanently bundles 57270 into 57260. That means you'll get a denial on the enterocele repair if you try to report these two codes for separate payment as you have suggested.
