Careful When Ordering Your Procedure Codes

Question: My ob-gyn performed the following procedures: anterior colporrhaphy with grafting, a posterior colporrhaphy with grafting, urethropexy by TVT, and a sacrospinous ligament suspension of the vagina. How should I code this?

Answer: You should report the following codes in this order, which are ranked from highest to lowest relative value units:

- 57288 (Sling operation for stress incontinence [e.g., fascia or synthetic])

- 57260-51 (Combined anteroposterior colporrhaphy; multiple procedures)

- 57282-51 (Colpopexy, vaginal; extra-peritoneal approach; multiple procedures)

- two units of +57267 (Insertion of mesh or other prosthesis for repair of pelvic floor defect, each site [anterior, posterior compartment], vaginal approach [list separately in addition to code for primary procedure]) with no modifier attached. You don't need a modifier on the two units of 57267 is because it is an add-on code.
