Which ICD-9 Codes You Should Use for US

Question: I-m trying to settle a problem. What is the basic diagnosis code everyone uses for 76811, level-two  ultrasound at 20 weeks? I-m using V28.3 and 655.83. Should I include anything else?

Answer: You need to identify the reason for the scan. If this ultrasound is a routine screening, you should use only V28.3 (Screening for malformation using ultrasonics).

If the physician has reason to believe there is a problem with the fetus, then use the 655 category code that identifies that reason. For instance, you might use 655.83 (Other known or suspected fetal abnormality, not elsewhere classified; antepartum condition or complication).
