Question: Our physicians are now using the DaVinci robot for laparoscopic procedures. How should we bill for a robot-assisted total laparoscopic hysterectomy, bilateral salpingo-oophorectomy?

Answer: You will not get paid extra for the robotic surgery, and if you press the point, you might even get the surgery denied as investigational. 

That said, you should bill a total laparoscopic hysterectomy using the unlisted-procedure code 58578, Unlisted laparoscopy procedure, uterus.

This procedure is more difficult than a laparoscopic assisted transvaginal hysterectomy (LAVH), so you want to capture as much reimbursement as you can. But you will not be paid extra for the -robotic- part of the surgery. Keep in mind: Your physician may take longer than normal if he is just learning how to use the DaVinci robot, but the learning curve is not a billable event.
