Question: Is 57267 the add-on vaginal mesh code?

Answer: Yes, this is the correct code. The descriptor contains the wording "... for repair of pelvic floor defect, each site (anterior, posterior compartment), vaginal approach." CPT intends for you to use this code with the anterior and posterior repairs, but you may find debate on whether you should use it for vaginal colpopexies. 

This is because CPT stipulates that you can only use +57267 (Insertion of mesh or other prosthesis for repair of pelvic floor defect, each site) with 45560 (Repair of rectocele [separate procedure]), 57250 (Posterior colporrhaphy, repair of rectocele with or without perineorrhaphy), 57260 (Combined anteroposterior colporrhaphy), and 57265 (... with enterocele repair).

For abdominal colpopexy, the ob-gyn always uses the mesh, so no additional codes apply.
