Question: My ob-gyn documented "lysis of adhesive (vertical band) in the vaginal cuff." How should I report this? Answer: You won't find any code that comes close to the procedure you describe, so you will have to use 58999 (Unlisted procedure, female genital system [nonobstetrical]) - the same code you would report for granulation of the vaginal cuff or bleeding of the cuff following hysterectomy. The answers for Reader Questions and You Be the Coder were provided by Melanie Witt, RN, CPC, MA, an ob-gyn coding expert based in Fredericksburg, Va.

None of the codes fit for excising anything other than a lesion from the vagina, and payers would not consider this procedure a colporrhaphy under the CPT definition.
Remember, you'll have to be extra vigilant with your documentation and provide a description of the procedure in plain English as well as a comparison to a procedure that has an existing code (if possible).
