Question: How should I code a twin pregnancy in which there is one normal fetus and one papyraceous fetus?

Answer: You should look at using 646.0x (Papyraceous fetus) along with 651.0x (Twin pregnancy). The fifth digit depends on the current episode of care. For example, a "1" indicates the patient delivered, with or without mention of antepartum condition, while a "3" means the patient has not delivered yet and is still in the antepartum period.
