Question: Which ICD-9 and CPT code should I use for a six months post-hysterectomy patient who underwent apical vaginal cuff granulation tissue removed with forceps and coagulated with silver nitrate?

Ohio Subscriber Answer: You have two ICD-9 coding options here, 623.8 (Other specified noninflammatory disorders of vagina) or 701.5 (Other abnormal granulation tissue).  The 701.5 code may be considered incorrect by some payers because it is located in the skin and subcutaneous tissue chapter of ICD-9.
 
The surgery code you should choose is 17250 (Chemical cauterization of granulation tissue [proud flesh, sinus or fistula]).
