You Can Use Male Infertility

Question: Can I use male infertility (606.8) on a claim for a woman's visit? I'm concerned that the claim will be denied with the male code when I report semen analysis without Huhner test, sperm washing and intrauterine insemination.

Massachusetts Subscriber Answer: You should be able to use 606.8 (Infertility due to extratesticular causes) for the sperm washing (58323, Sperm washing for artificial insemination), depending on the carrier's guidelines. You also may use it for the insemination (58322, Artificial insemination; intra-uterine). But you should report the semen analysis (89310, Semen analysis; motility and count [not including Huhner test]) with V26.21 (Fertility testing).
 
Some infertility specialists view infertility as a "couple" diagnosis. Consequently, you may want to present this interpretation to the payer if it denies the services for the diagnosis code submitted - rather than because it doesn't cover the service.
