Norplant Removal

Question: A patient was seen in the office for a Norplant removal (we did not insert the Norplant), but our physician was unable to do it. We sent the patient for an x-ray and requested records from the original physician. Our physician will then attempt to remove Norplant at a later date. How do I code for the failed attempt, and how will I code the visit when we reattempt to remove the Norplant?

Answer: For the initial attempt, use 11976-53 (removal, implantable contraceptive capsules [discontinued procedure]). Submit this claim with documentation as to the failed attempt, including sending the patient for an x-ray and requesting the original records. Then for the second attempt, code 11976-76 (removal, implantable contraceptive capsule [repeat procedure by same physician]).
