Modifier Key for E/M and D&C on Same Date

Question: Can I bill for an office visit and a suction dilation and curettage (D&C) for a missed abortion on the same date? Ohio Subscriber Answer: Yes, if the E/M service is separate and significant from the D&C. If this is the case, you should append modifier -25 (Significant, separately identifiable evaluation and management service by the same physician on the same day of the procedure or other service) to the E/M code to show that it is separate from the D&C. If the E/M service includes only a discussion of the procedure about to be performed or informed consent, you should not report the E/M service in addition to the D&C.
