How to Report Laminaria Plus Suction Curettage

Question: If the patient was given Laminaria on 07/27 in the office and then seen at the hospital on 07/27 for suction curettage would the suction curettage still be medical management? Or can we bill 59820?

Answer: If the purpose of the laminaria was to dilate the cervix in advance of performing the surgical removal of a missed abortion, you should report 59820 (Treatment of missed abortion, completed surgically; first trimester) for the surgical procedure. If the purpose of laminaria was to initiate labor, and the patient aborted some but not all of the products which then required surgical removal of the retained tissue, the CPT® code would change to 59812 (Treatment of incomplete abortion, any trimester, completed surgically) and the diagnosis would no longer be missed abortion (632 in ICD-9; O02.1 in ICD-10), but rather spontaneous incomplete abortion with hemorrhage (634.11 in ICD-9; O03.1 in ICD-10).
