Question: Would I use the unlisted CPT® code for this?

Procedure: Vestibular Anesthesia Testing
Indications: Vestibulitis
Procedure: Combination of benzocaine/lidocaine/tetracaine cream prepared by pharmacy, and brought to clinic by patient. Patient placed in lithotomy position, and cream applied to vestibule with cotton-tip applicator. Instructed to attempt vaginal insertion within 4-6 hrs after cream application, and notify us of result. Patient tolerated procedure well.

Answer: Billing this as an unlisted procedure is most likely going to result in a denial. Your better option is to bill an E/M service (99211-99215, Office or other outpatient procedure …) as this is in keeping with CPT® instructions in CPT® on topical applications.
