Code This Hysterectomy, TVT, and Cystoscopy Scenario

Question: My ob-gyn performed a Da Vinci supracervical hysterectomy with myomectomy, transobturator tape, obtryx sling, cystoscopy. How should I report this?South Carolina SubscriberAnswer: For the laparoscopic supracervical hysterectomy (which includes myomectomy, if performed), you should report 58541-58544 (Laparoscopy, surgical, supracervical hysterectomy ...). For the TVT, you should report 57288 (Sling operation for stress incontinence [e.g., fascia or synthetic]). Remember, a sling is a sling is a sling, and this was not a two sling procedure. You should bundle the cystoscopy into this sling procedure.  Finally, you can report S2900 (Surgical techniques requiring use of robotic surgical system [list separately in addition to code for primary procedure]) for the use of the Da Vinci robot, but you won't find any reimbursement attached to this code. No one will pay the surgeon extra for using the robot.
