Question: What code should I use for laparoscopy with biopsy of right ovarian cyst and removal of three small fallopian tube cysts? Massachusetts SubscriberAnswer: Code 58662 (Laparoscopy surgical; with fulguration or excisions of lesions of the ovary, pelvic viscera, or peritoneal surface by any method) will cover the removal of the fallopian tube cysts. The biopsy code is 49322 (Laparoscopy, surgical; with aspiration of cavity or cyst [e.g., ovarian cyst] [single or multiple]), but beware: some payers may bundle this.
