Answer: No. You don't need to do any additional assessment or OASIS data collection in this situation, says Wisconsin's Department of Health and Family Services in its OASIS frequently asked questions. The patient will have already had OASIS data reported at the point of transfer to the inpatient facility. All that is left to do is a "paper discharge" in the agency's records, DHFS advises.

Question #2: Can you discharge a pa-tient who remains in the hospital beyond the 60-day certification period? What information do you use to complete the discharge OASIS?

Answer: Yes, the agency should discharge a patient who remains in the hospital beyond day 60 of the episode, according to the Centers for Medicare & Medicaid Services' frequently asked questions. Because the patient has not been in the agency's care since admission to the hospital, no additional OASIS assessment is needed.
