Clinician's Struggle With M0460

Q: If a patient has several pressure ulcers at different stages and rates of healing, how do I decide which is most problematic in answering M0460?

A: This is one of those OASIS situations that requires both technical scoring information and clinical judgment, explains OASIS expert Linda Krulish with Redmond, WA-based Home Therapy Services. There is no cut-and-dried answer. It is not just the largest or the one in the most advanced stage, although those are important considerations. Also ask if one ulcer is worsening, while another is healing, she suggests. The location may make one more problematic than another, either because of difficulty in treating the area or difficulty in relieving pressure. The effect of one ulcer on the patient's activity may make it more problematic than another, she adds.
