Who Has The Right To Write In The Answers?

Must the clinician doing the OASIS assessment be the one to write in or mark the answers to all the OASIS items?

It's acceptable for clerical staff to record "preliminary responses" to some OASIS items, the Centers for Medicare & Medicaid Services explained in questions and answers from the 2003 OASIS Educators Conference. For example, the patient tracking sheet includes items such as address, zip code, payment source information and identification numbers that could be provided at the time of referral. When the assessing clinician signs the OASIS assessment, she is verifying the accuracy of all the information provided, including that entered by clerical staff, CMS says.
