Do you know when a grafted pressure ulcer becomes a surgical wound?

OASIS-C logic and ICD-9 coding logic don't always agree. That's certainly the case when it comes to pressure ulcer staging. When skin or muscle grafts are used to treat a pressure ulcer, you have to know when to consider the site a surgical wound and when it remains a pressure ulcer.
