Check Approach for Orbital Procedures

Question: Our surgeon removed a tumor at the orbital apex through a transcranial approach by raising an orbitofrontal flap and removing the orbital roof. How do we report this procedure?

Answer: You confirm the transorbital approach to the orbit as your surgeon does a craniotomy with removal of the roof of the orbit. Since your surgeon is removing a tumor, you report code 61333 (Exploration of orbit [transcranial approach]; with removal of lesion). If the operating microscope was used for microdissection, you may additionally report +69990 (Microsurgical techniques, requiring use of operating microscope [List separately in addition to code for primary procedure]).

Note: The transcranial approach includes the fronto-orbital, fronto temporo-orbital, subfrontal, or the pterional approaches.
