Use G40.5- for These Epileptic Seizure Patients

Question: Encounter notes indicate that a patient suffered from “epileptic seizures, alcohol-related.” Is there a specific ICD-10 code for this condition, or should I choose a generic epilepsy diagnosis code?

Answer: You should report alcohol-related epilepsy with G40.5- (Epileptic seizures related to external causes).

Per ICD-10, you should also use G40.5- for these epilepsy types:

  • Epileptic seizures related to drugs
  • Epileptic seizures related to hormonal changes
  • Epileptic seizures related to sleep deprivation
  • Epileptic seizures related to stress

Don’t stop there: Alcohol-related epileptic seizure diagnoses always carry a 5th character of “0” to indicate that the seizures are not intractable (according to ICD-10, all G40.5- seizures are not intractable). Then, you’ll need to choose a 6th character based on the presence or absence of status epilepticus. If you code to the highest point of specificity for alcohol-related epilepsy, you’ll choose from one of the following ICD-10 codes:

  • G40.501 (Epileptic seizures related to external causes, not intractable, with status epilepticus)
  • G40.509 (Epileptic seizures related to external causes, not intractable, without status epilepticus)
