Confirm Area of Excision in Lumbar Keloid

Question: For patient who had a LP shunt placed few months ago, our surgeon did an exploration and revision of LP shunt to adjust pressure of water. Before the shunt procedure, a lumbar keloid at the old incision site was removed. Does code 63744 (Replacement, irrigation or revision of lumbosubarachnoid shunt) include the exploration and revision of shunt? How do we report for the lumbar keloid?
Answer: Code 63744 is inclusive of any exploration and revision of the shunt. For lumbar keloid, the removal would be considered incidental to the shunt revision if the same incision was used.
