Question: Encounter notes indicate that the provider performed an electroencephalogram (EEG) for 112 minutes. I reported 95812 x 2 and received a denial. What did I do wrong? AAPC Forum Subscriber Answer: You should have chosen the next code listed in your CPT® manual: 95813 (Electroencephalogram (EEG) extended monitoring; 61-119 minutes). Remember to report the code that most accurately reflects your provider’s actions and total encounter time.
