Dupuytren’s Contracture Questions? We’ve Got Answers

Question: I’m stumped. Notes indicate that the provider performed collagenase injection for a patient with Dupuytren’s contracture. Can you help with a CPT® and ICD-10 code for this encounter?

Answer: For this encounter, report 20527 (Injection, enzyme (eg, collagenase), palmar fascial cord (ie, Dupuytren’s contracture)) for the injection with M72.0 (Palmar fascial fibromatosis [Dupuytren]) appended to represent Dupuytren’s contracture.

The injection: When the provider performs 20527, “the provider injects an enzyme, such as collagenase, into the palmar fascial cord as a therapeutic medical procedure to dissolve an abnormal tissue knot of the palmar fascia, or Dupuytren’s contracture,” reports Codify by AAPC.

The diagnosis: Dupuytren’s contracture — or palmar fascial fibromatosis — “refers to gradual thickening of tissue into a cordlike band below the skin of the palm which limits extension of the affected finger due to an autoimmune reaction,” according to Codify by AAPC.
