Try your hand at these 3 very different scenarios.
How would you code these three residents at G1d (how the person walks in the corridor on unit) for their ADL self-performance (Column A) and ADL support provided (Column B)?
Case No. 1: Mrs. Q requires continual verbal cuing and help with hand placement when walking down the unit hallway. Mrs. Q needs frequent reminders about how to use her walker, where to place her hands and to pick up her feet. She frequently needs to be physically guided to the day room.
Case No. 2: Mr. V requires two therapy staff and parallel bars to learn how to ambulate. He currently attends physical therapy six days a week. He uses a wheelchair on the nursing unit.
Case No. 3: A staff member must provide continual verbal cuing while the resident walks down the hallway to ensure he walks slowly and safely.
ANSWERS:
Case No. 1 (Mrs. Q): Self Performance = 2; Support Provided = 2
Coding rationale: Resident requires non-weight bearing assistance for safe ambulation.
Case No. 2 (Mr. V): Self Performance = 8; Support Provided = 8
Coding rationale: Activity did not occur.
Case No. 3: Self Performance = 1; Support Provided = 0
Coding rationale: Resident requires staff supervision, cuing and reminders.
